Interactions / Warnings:
THYROID TUMOUR WARNING (BLACK BOX — US label): Semaglutide caused dose-dependent thyroid C-cell tumours in rodent studies. Pregnancy interaction:

1. STRICT EMPTY STOMACH RULE — THE MOST IMPORTANT INSTRUCTION:
• Rybelsus MUST be taken first thing in the morning on a completely empty stomach with NO MORE THAN 120mL of plain water only. No coffee. No tea. No juice. No other liquids. No food. No other medicines. Then wait a FULL 30 MINUTES before your first meal or any other oral medication. Missing this rule can reduce semaglutide absorption by up to 90%, making the tablet ineffective. 2. MANAGE NAUSEA AT INITIATION — START LOW, GO SLOW:

4. THYROID SURVEILLANCE:
• Counsel patients to report any new lump in the neck, voice changes, or difficulty swallowing. While human thyroid risk is not confirmed, the precaution stands — especially in patients with thyroid nodules or family history of thyroid cancer.
5. DIABETIC RETINOPATHY — SCREEN BEFORE STARTING:
• Rapid HbA1c reduction (especially from very high baseline) can temporarily worsen diabetic retinopathy. Ophthalmology review is advisable before initiating Rybelsus in patients with known retinopathy.
